The Mid-Toronto Diabetes Education Program

The Mid-Toronto Diabetes Program provides free diabetes self-management and prevention education for adults with Type 2 diabetes, pre-diabetes, and for those at high risk of developing diabetes.

Managing your diabetes

Our Registered Nurse and Registered Dietitian facilitate group education, workshops and provide individual counselling sessions. Caregivers and family members are encouraged to attend.

Our skill-building sessions help you set realistic and achievable goals to self-manage your diabetes and reduce your risk of developing complications. The group education sessions and workshops may also be available offsite to accommodate the needs of the growing community.

Topics include:

  • Diabetes awareness and are you at risk?
  • Tips and benefits of blood sugar monitoring
  • Recognizing the signs and symptoms of high and low blood sugars
  • Healthy eating for diabetes prevention and management
  • Meal and snack planning for blood sugar control
  • Cholesterol management and heart healthy eating
  • Getting active and include exercise as part of your healthy lifestyle
  • Weight management
  • Reading food labels and grocery shopping tours
  • Diabetes medication management
  • Foot care assessments
  • Preventing long-term complications of diabetes

Foot Care Clinic

Is available on a weekly basis for registered participants of the diabetes program and includes a foot care assessment with a Registered Nurse. Please click here for more information.

We are here to help

By participating in the diabetes program, you will receive the tools and information to create a personalized care plan. We will help you understand the role of nutrition, exercise, medication, and blood sugar monitoring to self-manage and live a healthy and active life with diabetes.
